The Training Forms listed below are the most current and up-to-date versions available. It is therefore essential that training forms are continually obtained via the website rather than printed copies. Failure to do so is considered a breach of SLSNSW policy.

(Form 14)

To record when participants are ready to be assessed

 

Club to submit to branch (through SurfGuard) for the allocation of an assessor and assessment

Club to forward to Branch Secretary/ Administrator for endorsement. Branch to return with assessment

Manual Award Data in SurfGuard

(Form 80)

 

Manually add award data into SurfGuard.

 

Members to complete form and attach proof of an award such as a copy of the original certificate, an award number, copy of page from annual report or place and date attained.

Form is to be endorsed by the club and branch. SLSNSW will entered into SurfGuard.

Branch Education Committee Agenda

(QM200)

To provide a guide of the items that should be included in the Branch Education Committee Agenda and discussed at committee meetings.

Used by the Branch Education Committee as a template agenda for committee meetings.

 

Branch Education Committee Agendas should be retained by the branch inline with record keeping policies.

Improvement Request

(QM300)

To ensure that any problems are detected, identified, isolated and corrected in a prompt and isolated manner.

 

 

 

 

 

 

Completed by any member involved in a course who identifies any areas/resources/

SOPS that can or should be improved.

 

 

 

 

 

All trainers, assessors, facilitators should keep one form on them at all times in case an improvement is identified. Depending on Branch requirements, these forms may be sent to the Branch Education Officer for review by Branch Education Committee and then forwarded to the SLSNSW Manager, Education or may be submitted directly to the SLSNSW Manager, Education.

Participant Evaluation of Course

(QM303)

Used as a quality management tool with an aim to promote continuous improvement of the delivery of our courses.

 

Evaluations should be completed by all participants at the completion of a training course.

 

Used by all trainers and facilitators. All Participant Evaluation Forms to be attached to completed Training Course Report and sent to Branch Education Officer for review and summary.

Personal Evaluation Form

(QM304)

Used to identify personal strengths and weaknesses in the delivery of a training course or assessment.

 

 

All trainers, assessors and facilitators to complete form and attach to re-endorsement application form.

 

A minimum of one Personal Evaluation Form is to be sent into the branch with the relevant Re-endorsement forms for trainers, assessors and facilitators by 31st December.

Training Course Report

(QM305)

Gives an overview of the course, the participants, results achieved and recommendations for improvement.

 

 

Trainer and assessor or facilitator to fill in report at completion of each course.

 

 

 

Form to be sent to Branch Education Officer with course evaluation forms. Branch Education Officer to read evaluation forms and record general overview of feedback. To be reviewed by Branch Education Committee.

All course participants are to be shown this form and shown how to complete it by a trained RPL assessor.

 

 

All course participants are to be informed of the RPL process and given access to the RPL Application Form prior to, or at the start of the course. Form to be used when applicant believes they can gain RPL for the award.

Trained RPL assessor to help applicant to complete form. Form to be assessed by RPL assessor.

Opportunity for RPL applicants to provide feedback about the RPL process.

To be provided to RPL applicants.

Form should be submitted to the RPL assessor.

Record of Assessment Outcome (RPL)

(TA201)

To be completed by RPL assessor to record the RPL assessment outcome.

 

RPL assessor to complete once received RPL application and RPL interview has taken place.

 

Copies are to be given to the applicant, SLSNSW Manager, Education and branch. Branch to process award on SurfGuard through Form 14.

Culturally & Linguistically Diverse – Student Assessment Form

(TA300)

Private and confidential document that is used to identify participants with special needs who may require alternative/specific learning methods.

Trainers/ facilitators may use this if they identify in the beginning of a course that some participants may have learning difficulties.

Trainer/facilitator should keep these forms on them or at least have access to these forms in the surf club.

Participant Exit Questionnaire

(TA301)

To identify possible causes for participants leaving courses prior to completion and to identify possible areas for improvement.

 

 

Any participant leaving a course before completing it should fill in this form.

 

 

 

Completed forms should be read by trainer/facilitator for feedback and then attached to completed Training Course Report and sent to Branch Education Officer for review by Branch Education Committee.

To ensure all trainers, assessors and facilitators are suitably inducted.

Checklist is to be completed by all trainers, assessors and facilitators prior to undertaking the delivery or conduct of assessment.

Completed form to be filled in trainer and assessor file at branch. Form to be attached to facilitator application form and sent to SLSNSW.

Mentor Observation Checklist

(HR101)

Checklist used by mentor to observe the probationary trainer, assessor or facilitator and provide feedback on their performance.

Used by mentor to check probationary trainer, assessor or facilitator is competent before being recommended for endorsement.

Sent into the branch by the mentor or given to the mentoree to put with their application form which is then sent to the Branch Education Officer.

Registration form for trainers /assessors. Identifies what qualified trainers

/assessors clubs have and what awards they are eligible to train.

Probationary trainers /assessors to complete. Form to be signed by mentor and sent to Branch Education Officer.

Registration form for a trainer /assessor who wishes to continue to train in the new season.

Needs to be completed and sent to the Branch Education Officer by 31st December each year for assessors and every second year for trainers.

All trainers /assessors are required to send this form into the Branch Education Officer along with other forms associated with re-endorsement.  September 2009

Facilitator Application Form

(HR107)

Registration form for SLSNSW facilitators. Identifies what qualified facilitators the branch has and what awards they are eligible to facilitate.

Probationary facilitators to complete. Form to be signed by mentor and sent to Branch Education Officer.

Completed form sent to Branch Education Officer for approval from the Branch Education Committee before sending to SLSNSW for endorsement.

Registration form for a facilitator who wishes to continue to facilitate in the new season.

Needs to be completed and sent to the Branch Education Officer with your VET logbook by 31st December each year.

Branch Education Officer to check form & VET logbook for 100 points. Branch Education Committee to approve and then send form to SLSNSW for endorsement.

(HR109)

Used as a tool to document the review of trainer/assessor/ facilitator performance and provide invaluable feedback.

Used during any training/assessment session.

 

A copy to be retained by the trainer/ ssessor /facilitator receiving the review and the original sent to Branch Education Officer.

Internal Proficiency Validation Form

(HR111)

Document to be used during club and branch internal audits.

 

 

Internal club and branch audits are to be completed a minimum of once per year using this report.

 

Club Captain/Chief Training Officer responsible for overseeing club audit. Branch Education Officer responsible for overseeing branch audit.

(HR115)

Used prior to Trainer/Assessor and Facilitator Application Forms to obtain details of interested parties and establish an endorsement plan

To be completed by members interested in becoming a Trainer, Assessor and/or Facilitator

 

The form should be returned to the Branch Education Officer for action.

Venue Checklist

(HR200)

Used to identify a safe learning environment (applies to both internal and external venues).

 

 

 

One form for each training and assessment venue is required to be filled out originally or when construction changes have occurred.

 

 

Responsibility of Chief Training Officer to complete venue checklist for club courses, forms to be filed at club.

Responsibility of Branch Education Officer to complete venue checklist for branch courses, to be filed at branch.

Hazard Record Form

(HR201)

To record any hazards identified in the training environment so corrective and/or preventative action can be taken.

When a hazard or possible risk is identified (i.e. broken step).

Trainers, assessors, facilitators to complete and forward to club/branch management to rectify.

To record participant attendance for courses that are held over multiple sessions.

Attendance records must be recorded at every training session for each course.

Course Enquiry Form

(AD200)

Planning tool to assist clubs in knowing what awards their club members are interested in obtaining so they can organise those courses.

Write in the interested persons contact details whenever there is an enquiry about a course. Use in conjunction with Training Management Plan to develop training calendar.

Used by all club members involved in training or administration of training courses. Should be kept in an easily assessable, central location in the club.

To gain sufficient information regarding participants and to ensure participants understand and agree to SLSNSW policies.

Trainers and facilitators must ensure all participants complete the enrolment form in its entirety at the start of each course.

Completed form is to be kept with all course information and documents.

Award Pre-requisite Confirmation

Form provides confirmation member holds pre-requisite awards.

Members to complete form if wish to complete training at a branch other than their own.
